About Power of Attorney Map

Power of Attorney Map is a free, plain-English guide for people in the United States who want to name someone to handle their money or health care if they cannot, for agents and families who already have to act, and for families looking at guardianship or conservatorship. For each state it explains what the documents are called, the official forms, how to sign them (notary and witnesses), whether a power of attorney is durable, how to revoke it, who decides when there is no directive, and where to find free legal help. Free tools make a signing checklist and the letters to revoke a power of attorney or a health care directive, in your browser.

We are not a law firm, not a court and not a government agency. We do not prepare your documents, we never pre-fill official forms and we never suggest which powers to give or whom to choose. We do not tell you whether your document is valid: only a lawyer or a court can do that. We link to official sources: state law, state courts and their self-help pages, state agencies and legal aid.
